Your GT660 may not support sufficiently high HDMI bandwidth to drive it at
4K. Possibly your cable likewise - you want a 'high speed' HDMI cable.
It is possible the display only handles 4K using HDMI 2.0 and the GT660
doesn't have that.
Although I have successfully driven a 4K display from a GTX650 with HDMI 1.4
at 30Hz (well, 29.94Hz or something), which may be an option. That may
require a custom mode.
