    Apologies if I'm missing something, but instead of this patch, why
    can't Mutt users turn on:

    QP_REQUIRED = 1

    I hadn't considered QP_REQUIRED, but the difference AIUI is that
    QP_REQUIRED encodes the whole message, while this patch just takes care
    of the header fields, which are not specified to support 8-bit encoding,
    and leaves the body alone. Most MUAs can deal with an un-encoded body.
