This is what Talkatone support sent back.
I have to ask the recipient to test it out but it doesn't say that the Talkatone app has to be awake to ring upon receiving a call.

1] What you are hearing when you call the other party is the generic
outbound voicemail greeting. The recipient can change the generic Talkatone voicemail greeting to a custom greeting.
The custom voicemail greeting option allows you to record your own
personalized voicemail recording.
1. First, go to the setting menu.
2. Next, scroll down and select the Voicemail Greeting option.
3. Then, select the Custom option.
4. Next, select Record and begin recording your custom voicemail greeting.
You can select the Play button to hear an audio playback of your recording.
5. If you are satisfied with your recording select Done to save your new
custom voicemail greeting.
6. Lastly, restart the app, so your changes can be saved and reflected on
the app.

2] For the recipient's iOS device:
If you don't receive sound for incoming calls/texts, please try to verify
the following:
On your iDevice settings, please make sure that the Talkatone app is under Notifications, and the Allow Notifications feature is ON. Please make sure
all buttons are enabled. (see video Tutorial below)
If this doesn't help. Please check your volume settings on the device
itself.
When on Talkatone, try the buttons on the side of your device, this should work.
If you are using an iPad make sure it is not muted. Please follow this link (
http://support.apple.com/kb/HT4085) to Apple's support pages for
instructions on how to fix this.
Alternatively try deleting the app and reinstalling it again and restart
your device.

They should try changing the current Talkatone ringtone to another
Talkatone ringtone. The current one may have a bad audio file.
You can change the current Talkatone ringtone under Settings (Advanced
Settings if applicable)/Notification and Sounds/Ring Tone.

3] In using our service, please bear in mind that call connections are
highly dependent on internet/data connections. Therefore, in areas of poor connections, you may experience connectivity issues affecting audio quality
and app performance.
Often deleting/reinstalling the app and restarting your device will correct
the issue.
