Since I`m not a conspiracy hobbyist trying to exploit information to confirm my biases of course I can think "Well, it was a sneak attack lasting seconds, Chaney caught a glimpse of this and a glimpse of that, and he is trying to quickly get a handleon the situation, so from his vantage when he saw the wound Kennedy received, he might have gotten the impression that it was on his face".
